OSHA Injury Reports

This OSHA Injury Report was compiled directly from data that is publicly available from the Occupational Safety and Health Administration (OSHA). OSHA Injury Reports can be helpful in understanding the types of workplace injury claims involving defective products or malfunctioning equipment in the workplace. 

The manufacturers of defective equipment or machinery resulting in a worker injury may be found to be negligent and liable for damages in a separate civil lawsuit. You may be able to recover damages on top of a workers’ comp claim if injured in a job accident caused, in part, by a third party. These third party personal injury cases should be given responsible legal guidance.

Report: Caught in running equipment or machinery during maintenance, cleaning at CHAMPION COOLER CORPORATION in Denison, Texas

An employee was performing maintenance on the trough machine. The guard was removed to ensure the machine was operating properly. One of the troughs fell out of the machine and activated the micro-switch, which activated the machine cycle. The employee’s left forearm was caught and injured in the machine.

An employee sustained burns to the side of the face and neck after coming in contact with heavy oil while unplugging a bleeder valve.

On 2/2/16, at 9:45 a.m., an employee was pouring hot water from a bucket into the sink when hot water contacted the employee’s right leg. The employee suffered 3rd degree burns from the knee to the ankle and was hospitalized.

An employee was getting ready to reposition portable source lights using a material lift cart. He picked up the front of the lift. As he twisted around, he turned/rolled his ankle and it gave out. He suffered torn ankle ligaments.

An employee was attempting to pump water into a natural gas line. After he was given the “all clear” sign by the pipeline supervisor, he opened a water valve and was instantly struck and injured by pressurized water. The gas line was not de-energized at the time.
