Report: Caught in running equipment or machinery during maintenance, cleaning at CHAMPION COOLER CORPORATION in Denison, Texas

An employee was performing maintenance on the trough machine. The guard was removed to ensure the machine was operating properly. One of the troughs fell out of the machine and activated the micro-switch, which activated the machine cycle. The employee’s left forearm was caught and injured in the machine.
