An employee was replacing a new liquid propane tank for a forklift. The propane leaked through a valve from the tank and onto the employee’s right index and middle fingers, burning them.

An employee was performing maintenance on a photo tool. Afterward, he added refrigerant to the tool, which began to leak. He attempted to stop the leak and contacted the refrigerant, burning both of his hands.
