Fractures

An employee was power washing the shop when he was struck by a reversing forklift that ran over and broke his right leg.

An employee was approaching the entrance end of a car wash when she slipped and fell on the wet surface (diamond plate), fracturing her left ankle and tibia. She was hospitalized.

An employee was attempting to repair a vibration pad. While working on the pad, the employee was sitting on the battery box. When the employee connected two wires, the vibration pad raised up and pinched the employee’s ankle, breaking it.

An employee was attempting to manually wipe excess lubricant off of elevator cables when the car moved, putting the injured employee’s arm in the pinch point between the ropes and the sheaves. The employee was hospitalized with a severe fracture and laceration to the left arm.

An employee was working on a frac tree. The tree fell, catching the employee on the leg and breaking it.

An employee was chasing individuals on foot when the employee fell 10 feet onto gravel ground, suffering a broken jaw and nose.

An employee was operating a horizontal lathe when he attempted to change the tooling on the tail stock. The moving chuck caught his tooling, causing the tooling to rotate and strike his left hand. He broke his left hand and received several lacerations on his two middle fingers that required stitches.

A forklift ran over an employee’s left foot, breaking it. He was hospitalized.

An employee was on a step stool. It moved and the employee fell 3 feet to the ground, suffering a broken left leg.

An employee was replacing hydraulic oil on a lift conveyor when the lift fell on his right elbow. He was hospitalized with a broken elbow.
