Report: Struck by swinging part of powered vehicle at Forterra Pipe & Precast in Houston, Texas

On March 01, 2017, a forklift driver was trying to lift a base pallet so the injured employee could place a 4X4 wooden board under it. The forks slipped off the base pallet and struck the employee’s jaw. The employee suffered a broken/dislocated jaw.
