Report: Struck by powered vehicle-nontransport, n.e.c. at Performance Food Group Inc. in Temple, Texas

While lifting a pallet of cases, three cases fell off the pallet. The forklift operator picked up two cases and stacked them on the dash of the standing forklift. When he went to pick up the third case, the other two cases fell over engaging both the dead man’s switch and the joystick, causing the forklift to rotate pinning him against an adjacent forklift.
