Two employees were changing out a dock leveler’s dock plate. While the injured employee was on his hands and knees under the leveler, installing the keeper-pins, a forklift that had been holding up the dock leveler’s lip hinge moved back and released the dock plate. The plate fell on the injured employee, breaking most of his ribs, causing internal bleeding, and causing a T-Fracture in his lower back as well as fracture to the upper back.
