Two employees were installing a 25-foot, 1000-watt halide light onto anchor brackets using a forklift when the metal fixture broke at the knuckle, striking one employee in the head and the other in the back. The first employee was hospitalized with a head injury, and the second was hospitalized with fractured vertebrae.
