Report: Struck by dislodged flying object, particle at Tri-Element Inc. in Gonzales, Texas

An employee was retrieving mud flaps to reinstall on a truck. A cotter pin on the winch line, which was secured to the rear of the truck, came loose. The wire on the driver’s side became unsecure and struck the employee on the left side, below the shoulder and back.
