Report: Struck by dislodged flying object, particle at WD Chips, LLC in Evadale, Texas

DATE: 08/31/16

An employee was assisting two other employee to remove a cylinder from a log loader. The employee was using a pry bar to assist the other employees that were trying to remove a pin. Once the pin was removed, the cylinder slipped forward in the mud causing the pry bar to jerk backwards and strike the employee in the head just below his hard hat knocking him unconscious. The employee suffered from a concussion and a small brain bleed. He was hospitalized.
