Report: Struck by dislodged flying object, particle at Millard Refrigerated Services, LLC in Sunnyvale, Texas

A forklift was used to attempt to remove a stuck electric pallet truck from inside a trailer. One end of a towing strap was attached to the forklift carriage and the other end was attached to the battery plate on the pallet truck. When the forklift operator backed up, the battery plate came off the pallet truck and struck an employee who was standing nearby in the head.
