An employee reached for cases on a second level pick slot, grabbed the product and sat it onto their pallet jack while standing on the pallet jack platform. The employee then lost their footing and tried to catch themselves by grabbing the control handle. The directional control was triggered in the forward direction. Their right foot was struck-by the platform and pinned against a rack leg. The employee sustained a laceration above the right ankle and a crushing injury to the right foot.
