OSHA Injury Reports

This OSHA Injury Report was compiled directly from data that is publicly available from the Occupational Safety and Health Administration (OSHA). OSHA Injury Reports can be helpful in understanding the types of workplace injury claims involving defective products or malfunctioning equipment in the workplace. 

The manufacturers of defective equipment or machinery resulting in a worker injury may be found to be negligent and liable for damages in a separate civil lawsuit. You may be able to recover damages on top of a workers’ comp claim if injured in a job accident caused, in part, by a third party. Report: Small-scale (limited) fire at Anthony Oak Flooring, Inc. in Magnolia, Arkansas

An employee was running a ripsaw and doing minor maintenance. The shaving bin caught fire. When it was opened, the shavings dropped out and burned the employee’s forearms. He was hospitalized.

Report: Other fall to lower level 11 to 15 feet at Foamtec International, LLC Warehouse in Waco, Texas

An employee was on a platform with a removable railing (designed to allow forklifts to bring down pallets with boxes of product). As the employee put the last box on the pallet closest to the edge, he fell off the platform, landing on the concrete floor 11.5 feet below and fracturing a heel.

The injured employee used a step ladder to climb on top of a mold. Another employee was on the step ladder assisting him and handing him tools as needed. The injured employee was startled and fell approximately 7 feet from the mold to the concrete floor, sustaining head and back injuries.

At around 1:20 p.m. on November 26, 2019, an employee was pulling an empty cardboard box out from a combo dumper by the plastic tie binding. The binding broke, causing him to fall forward onto the concrete floor and suffer a broken right femur. He was hospitalized.

An employee was connecting a nitrogen hose to a main line. The gasket inside the hose fell out. While the employee was trying to close the valve on the process side, PTBP crude sprayed the employee, causing second-degree burns to the neck and the hand.
