An employee was running a ripsaw and doing minor maintenance. The shaving bin caught fire. When it was opened, the shavings dropped out and burned the employee’s forearms. He was hospitalized.

An employee was on a platform with a removable railing (designed to allow forklifts to bring down pallets with boxes of product). As the employee put the last box on the pallet closest to the edge, he fell off the platform, landing on the concrete floor 11.5 feet below and fracturing a heel.
