OSHA Injury Reports

This OSHA Injury Report was compiled directly from data that is publicly available from the Occupational Safety and Health Administration (OSHA). OSHA Injury Reports can be helpful in understanding the types of workplace injury claims involving defective products or malfunctioning equipment in the workplace.

Report: Fall on same level due to slipping at PIGGLY WIGGLY SOUTH OPELOUSAS, LLC in Opelousas, Louisiana

An employee was wrapping meat using an automatic wrapper and putting the meat into a tall/mini shelving rack in the refrigerated market/area. The employee then transported the meat to the sale floor area. On her way back to the market area, the employee slipped and fell to the floor. The floor was wet from condensation.

A fire suppression system inadvertently went off, causing a white cloud. Employees working on a scaffold, 24 feet above the ground, scrambled to avoid the cloud. While descending a ladder, an employee slipped and fell 10 to 12 feet.
