OSHA Injury Reports

This OSHA Injury Report was compiled directly from data that is publicly available from the Occupational Safety and Health Administration (OSHA). OSHA Injury Reports can be helpful in understanding the types of workplace injury claims involving defective products or malfunctioning equipment in the workplace. 

The manufacturers of defective equipment or machinery resulting in a worker injury may be found to be negligent and liable for damages in a separate civil lawsuit. You may be able to recover damages on top of a workers’ comp claim if injured in a job accident caused, in part, by a third party. These third party personal injury cases should be given responsible legal guidance.

Report: Fall through surface or existing opening 11 to 15 feet at Allied Waste Services of Fort Worth, LLC in Fort Worth, Texas

An employee fell through an opening 15 feet to the ground. The employee broke the left hip, ribs, a tibia, and sustained damage to the spleen. The employee was admitted to the hospital and underwent surgery.

Report: Small-scale (limited) fire at The Southwestern Plating Company in Houston, Texas

Maintenance employee for Southwestern Plating was working inside a circuit breaker box that was not locked out. A flash fire occurred, burning the employee’s arms (upper and lower), face, neck, and torso. The employee was rushed to Memorial Hermann Hospital (Burn Unit) where he is currently hospitalized.

On 1/27/15, the employee was getting a cat out of a carrier with the assistance of another person. The other person was holding the box and she was getting the cat out. The cat bit her hand. After it continued to swell, the employee was admitted on 1/28/15 overnight for IV antibiotics.

Employee opened up a strainer within a processing line that was not de-energized, causing approximately 2000 gallons of TDI to spill out. The employee and two others were sent to the hospital. Two were treated and released, while the employee directly exposed was hospitalized.
