OSHA Injury Reports

This OSHA Injury Report was compiled directly from data that is publicly available from the Occupational Safety and Health Administration (OSHA). OSHA Injury Reports can be helpful in understanding the types of workplace injury claims involving defective products or malfunctioning equipment in the workplace. 

The manufacturers of defective equipment or machinery resulting in a worker injury may be found to be negligent and liable for damages in a separate civil lawsuit. You may be able to recover damages on top of a workers’ comp claim if injured in a job accident caused, in part, by a third party. These third party personal injury cases should be given responsible legal guidance.

An employee was installing batten bars onto the sheeting panel during the installation of an aluminum dump. His safety harness rope got caught in the aluminum panel and while he was trying to free it, he fell roughly 20 feet to the ground.

An employee and a couple of other guys were removing a 12 inch section of cargo pipe and a 12 inch valve with a crane. When they lifted the section of pipe, the section swung and the valve hit an employee. He fell to the deck (on the same level) and was injured.

Report: Struck by swinging part of powered vehicle at Dril-Quip, Inc. in Houston, Texas

A mechanic was at machine 4L-2 in the rough machine center when a machinist asked to borrow the 10-ton overhead gantry crane. As the machinist moved the crane, the hook struck the mechanic in the head. Later, the employee had a headache and blurry vision. He was taken to the hospital and admitted.

An employee was in a classroom working with toddlers when she slipped on a manipulative toy (used as a teaching aid) and hit her left knee on the floor. She was unable to stand-up and was hospitalized overnight.

On or about 16 April 2015 at approximately 1530 hrs an employee was leveling a section of the construction site with a 2007 Ingersoll Rand SD77DX Vibratory Smooth Drum Roller, when the vehicle started to slide down the embankment. The employee dismounted the vehicle and injured his neck upon impact with the ground.
