An employee was pulling a plate with a come-a-long inside a pipe vessel. A forklift was used as an anchor and stopping point for the vessel. The plate became lodged in the vessel and the next time the employee pulled, the vessel moved and struck the employee on the right side of his body.

An employee operating a forklift machine went over to pick up a pallet from the floor. The employee was not aware that the machine was jammed. The next pallet that came out of the machine fell on the employee.

While a boom was being unlatched at a well site, the boom snapped and threw the pipe off, hitting an employee on the ground.

An employee was in ladder training. He climbed 18 feet to the top of the pole. While coming down the ladder, the ladder turned and began to slide off the pole. The employee fell about 15 feet.

An employee was struck by and ran over by a forklift.
