An employee was using a crow bar to move steel beams for unloading while standing on the trailer bed of a truck.

The crow bar came loose from the material, causing the employee to lose his balance and fall to the ground. He extended his arms to break his fall, resulting in broken bones in his right wrist and left elbow. He required surgery.
