An employee was preparing to install a fire alarm part while standing on a 6-foot A-frame ladder (second rung from the top). He was attempting to get a marker from his pocket when the ladder twisted and he fell to the pavement, injuring his pancreas and requiring hospitalization. His right leg also became caught up in the ladder, causing dislocation and damage to the knee.
