Two employees were working in a man-made basket that was tied to the clamps and mast of a powered industrial truck. The basket was raised 7 feet when a safety attachment broke and both employees fell to the ground. One employee was hospitalized with a fractured right elbow, fractured right ribs, and a punctured right lung.
