On 9/30/19, an employee went up the ladder of a separator tank. He was on top of the ladder when a pop-off valve released. He was exposed to gas and lost consciousness, falling 20 to 30 feet to the ground. He was hospitalized with a fractured left fibula, fractured right hip, and ruptured kidney.
