An employee was working on commercial roof to pick up a debris and materials. He picked up a piece of plywood put there by another contractor. He did not know it was covering a 4′ x 4′ hole, stepped in it, and fell 20 feet to the concrete below, breaking his right foot and requiring stitches on his left leg.
