Report: Fall or jump from and struck by same vehicle in normal operation, nonroadway at Stine LLC in Sulphur, Louisiana

An employee took a forklift off a truck to load lumber. He drove the forklift to the driver’s side of the truck and parked it. He left the seat belt buckled. He had one foot on the forklift and the other on the truck bed when he reached over to grab a pail of roofing nails. His foot then hit the gas pedal on the forklift, causing it to go with him still straddled between the forklift and the truck bed. He then fell to the road and was run over by the forklift, injuring his hips and legs.
