An employee was driving a bulldozer. When it drove on an unlevel surface (about 2 feet difference), his foot slipped off the gas pedal and he fell out of the cab hitting his mouth and nose on the side mirror. The employee fell on the tracks of the bulldozer and it ran over his arm and leg resulting in a fractured femur.
