An employee was replacing a valve on a vacuum tank and was attempting to remove two bolts using an air grinder. As he started grinding the first bolt, a flash explosion occurred. He suffered a cut on his head; received third degree burns to his hands, forearms, and stomach; and fractured his left leg. He was hospitalized and required surgery.
