On May 16, 2017, at approximately 10:55 AM, an employee was putting protectors on the end of a pipe when it became misaligned on the table. The thread inspector was tasking the next step on the process line by actuating the mechanical control switch to prepare a pipe for inspection. The employee noticed that the pipe was rolling back towards the thread inspector and pushed the thread inspector away from the pipe. The device (or kicker) that allows pipes to roll was left in the up position, and a second pipe rolled toward the employee, whose left hand was caught between the first and second pipes. The employee suffered an open fracture to the left ring and pinky fingers.
