Report: Caught in running equipment or machinery during maintenance, cleaning at Thelin Recycling Company, LP in Frisco, Texas

An employee was performing routine maintenance on a discharge conveyor belt when another employee activated the machine. The employee was then thrown into the moving conveyor, breaking several ribs. The machine was not locked/tagged out at the time of the incident.
