An employee was on the ground level manually lifting the dock plate to sit on a trailer. The dock leveler was a mechanical chain driven dock leveler that would not raise completely due to a mechanical issue with the tensioner. As the employee lifted the dock plate, he was pinned between the dock plate and the trailer resulting in broken ribs and a lacerated spleen.
