Soreness, pain, hurt-nonspecified injury

An employee was operating an ATV when it rolled and injured his left wrist.

The employee was opening a valve. The free end of a connected hose struck the employee when he opened the valve injuring his lower left leg.

On September 30, 2016, at approximately 11:00 a.m., an employee was attempting to help fellow crew members fuel up machinery when he lost his balance and fell. He received minor scrapes to his face and injured his right eye. He was hospitalized for surgery.

During rig down procedures, an employee was moving a mud tank with a tandem truck when an overhead walkway slid, hit the ground, and then struck the employee on the left knee.

An employee’s glove was caught in a conveyor belt pulley, injuring the employee’s right forearm.

An employee’s right foot was struck by an outrigger, injuring the big toe and second toe and requiring hospitalization.

An employee was using a lathe to machine a part, which was approximately 6″ wide, 5″ long, and 16 pounds, when the part came out of the lathe and impacted his abdomen.

An employee was helping to unload a 200-pound flare stack from a gooseneck trailer when the stack shifted and fell, striking the employee in the right shoulder and lower back area.

While descending a step ladder, an employee missed a step and fell, suffering a left ankle injury.

An employee was helping to install an 8-inch water line, working approximately 4-feet, 8-inches deep, when the dirt underneath the concrete of the street gave way and hit the employee on the right-side hip/pelvis.
