Soreness, pain, hurt-nonspecified injury

Around 1:00 p.m. on 11/18/16, an employee was leaving the repackaging station when she tripped over a metal piece of the rolling warehouse steel. She fell on her right hip and was unable to get up and walk, requiring hospitalization.

An employee was part of a rigging crew tasked with using an overhead 8-ton crane to move a heat exchanger through two columns. The employee pushed the exchanger through by hand and it swung back and struck the employee’s left index finger and thumb. The employee was hospitalized for surgery.

While working in a straight boom lift that was 5 feet off the ground, an employee bounced out of the bucket. The employee’s harness stopped the fall, but the employee suffered torso and rib injuries.

On November 16, 2016, at approximately 7:15 PM, an employee’s left hand entered a piece of moving equipment as it was coming to a stop and was injured. He was hospitalized.
