Soreness, pain, hurt-nonspecified injury

An employee was standing on a rig floor when equipment fell off a mast as it was erected and struck the employee, causing significant head and neck injuries. The employee was hospitalized.

An employee was driving a forklift during forklift certification and practical evaluations when the employee lost control of the forklift and hit a pallet racking system. The employee’s leg was outside of the forklift at the time and suffered an injury, requiring hospitalization.
