An employee was crimping wire terminals using a crimping machine. As she was fixing a jam, the machine crushed a finger on her left hand, causing the amputation of the tip. The machine was guarded at the time.

Rotating shaft, caught-in hazard located in the bakery section. The hazard was located at the conveyors.
