An employee was cutting billets with a propane and oxygen torch when sparks sprayed his left side. His undershirt caught on fire and he suffered second degree burns to the left side of his back.

An employee was looking at the orientation of a sprocket on a rotary machine when his right arm was trapped by a fixture on the machine. He suffered a cut/contusion/bruise on the inside of his arm above and below the elbow, requiring hospitalization.
