An employee on a ladder was caulking underneath eaves when the ladder slipped and the employee fell. The employee was approximately 12 to 14 feet from the lower level at the time. The employee was hospitalized with a partially collapsed lung and fractured wrist.

An employee was driving a forklift, traveling to his next pick-up, when the road embankment gave way beneath his tires and caused the forklift to turn over. He broke nine ribs, punctured a lung, and lacerated his forehead and right elbow.
