Fractures

An employee was using a torque multiplier ratchet wrench to loosen a bolt. The wrench broke and struck him in the arm, breaking the forearm. He was hospitalized and underwent surgery.

An employee was unrolling waterproof conduit on a roof. He tripped over a fall protection anchor installed in the roof and fell backwards onto a 3-inch plumbing vent pipe that extended 14 inches above the roof. He suffered several cracked ribs.

An employee was rolling a pole on a trailer to unload it when the pole fell from the trailer and struck the employee’s left leg, resulting in a broken tibia/fibula.

An employee was climbing an extension ladder to install metal wall panels at a residential building. The employee fell off the ladder and landed on the ground. He sustained a broken right femur and a laceration to the head.

A tree fell onto an employee’s head after being cut down. The employee sustained fractures to vertebrae T2 and T4.

An employee was transferring product from one truck to another using a transfer plate. The employee slipped and the transfer plate broke the employee’s left leg.

An employee was adjusting a pallet when it fell on his leg, breaking it.

An employee had climbed onto the roof to perform a task. When the employee was coming down the ladder, the ladder came out from underneath him and he fell to the ground. The employee sustained an ankle fracture and five broken ribs. He also sustained bumps and bruises.

An employee was moving a shopping cart out of the office and tripped, resulting in a fracture to her right foot.

While picking up equipment outside, an employee slipped on a wet, wooden deck and fell. The employee sustained a broken leg.
