Fractures

An employee was operating a forklift to move pallets out of a slot when his left calf was pinned and fractured between the forklift and a rack. His foot was outside the forklift at the time of the incident. He was hospitalized.

An employee was doing maintenance work on a flipover. While bolts were being removed, the inner frame of the flipover fell and landed on the employee’s foot, breaking it. The employee was hospitalized, requiring surgery.

An employee was painting the exterior of a home from a 12-foot ladder. The employee fell from the ladder and landed on a patio, suffering multiple broken ribs and a large gash on the head. He was hospitalized.

An employee fell from a metal roof that was being removed and replaced. The employee landed on a concrete floor about 22 feet below, suffering skull and neck fractures.

An employee was applying edge protectors on top of a load of drywall on a flatbed truck. He fell to the ground, suffering broken vertebrae and bleeding under the skin.

An employee was helping to move a hay trailer on a dolly. The hay trailer fell off the dolly and struck the employee between the knee and upper part of the leg resulting in hospitalization with a possible fracture.

An employee was standing near a shaft while it was being moved with an overhead crane. The shaft bumped into another shaft which hit the employee’s foot. His right foot was fractured.

An employee was assisting a forklift operator move an oxy/acetylene cutting rig. The employee’s hand got caught between a fork and the lifting eye on the cutting rig, causing a fracture to the index metacarpal bone in the hand.

An employee was unloading a super sack that was hoisted by a forklift into a hopper when the loops that hold the super sack up broke, causing the sack to fall onto the employee. The employee suffered a broken leg requiring surgery.

An employee was conducting an emissions test of a nitrogen vaporizer unit inside an instrument test trailer when a pipe explosion occurred outside. Pieces of the steel pipe scattered in all directions, including toward the trailer. As a result, the employee suffered a fractured right radius bone, requiring hospitalization and surgery.
