Fractures (except skull fractures) and concussions

An employee was in the SkyTrak personnel basket cutting tree limbs. The operator was repositioning the machine when it hit a mound of dirt and caused the SkyTrak to tip over. The employee jumped out of the basket and suffered a concussion and a broken right ankle.

An employee was on a 10-foot A-Frame ladder grinding a 3-inch carbon steel pipe with a 4.5-inch grinder. The employee fell off the ladder, breaking the left arm in three places (surgery required) and suffering a mild concussion.

A crane was removing a heater from a platform. The heater was lifted up to tighten the chains before resuming the lift, but the heater stack (7 ft. pipe) broke off of the heater. The broken piece struck an employee, who suffered a severe concussion and broken bones in the neck.

An employee was installing a sign while working from an extension ladder. He reached behind the ladder to stick vinyl onto the wall. He slipped and fell approximately 8 feet from the ladder onto the ground, sustaining a concussion, fractured left leg, and cut on his right knee.
