An employee was on a ladder winding the tension spring for an overhead door when the tension spring blew apart, causing the employee to fall off the ladder. The employee suffered shattered heels and a back injury.

An employee was on an extension ladder approximately 20 feet high changing a light bulb. The employee fell from the ladder onto the cement floor and suffered a broken shoulder and possible brain injuries.

An employee went upstairs to take measurements of the attic area. He stepped back and fell off of the open floor to the tile beneath. He suffered a fractured vertebra and was bleeding from the head. He was hospitalized.
