An employee servicing a package boiler was on the forks of a forklift, attempting to move pipe. He fell approximately 8 feet off the forks and onto the concrete, suffering a head laceration (that required surgery), a broken left ring finger, three chipped vertebrae, and a potential concussion.

An employee was helping to unload pipe from a trailer. When he attempted to roll the pipe, his left knee was pinned between a pipe stake and a trailer. He pushed the pipe back and dislodged himself. He suffered a deep knee laceration, tendon damage, and a kneecap fracture.

An employee was pushing a rolling gate closed while his right hand was on top of the gate. His right index finger was caught under the roller, breaking the finger and cutting it down to the bone.

An employee was collecting shopping carts in the company’s parking lot. While crossing the street to retrieve a cart, the employee was struck by a vehicle and suffered a right leg fracture, torn ACL, broken teeth and three fractured vertebrae.

An employee was troubleshooting a light fixture when he came into contact with an energized circuit. He was working from a scissor lift at a height of approximately 19 feet. After the shock, he fell out of the scissor lift and landed on the concrete surface below, suffering multiple fractures and requiring hospitalization.

An employee was operating an angle cutting machine. When the employee tried to remove a piece of metal, the machine began another cycle and the metal plate on the machine struck the employee’s wrist causing a severe laceration, severed tendons, and broken bones.
