Fractures and dislocations

An employee was working in a warehouse. A forklift pushed a load forward and the employee was trapped between the load and another forklift, suffering a fractured right foot and a dislocated toe.

An employee was in a derrick, making a connection. The employee’s right thumb was caught and crushed between the drill pipe and the elevator, suffering an open fracture/dislocation.

An employee was handling a racking board section. When the employee knocked a pin out of the racking board section, the racking board fell and hit the employee’s left leg. The employee suffered a dislocated hip and fractured pelvis, requiring hospitalization. The racking board was not properly tied off at the time of the incident.

An employee was making beds in a room at a senior living facility when she tripped over the corner of a bed, causing her to fall to the floor. She suffered a dislocated and fractured right shoulder.

An employee was filling up a truck at a gas station when he tripped over the gas pump hose and fell to the ground, suffering a broken collarbone and dislocated shoulder.

An employee was diagnosing an electrical issue on a skid steer when the unit knocked the employee to the ground and rolled up onto his leg. The employee sustained a dislocated left ankle, fracture left knee, and muscle damage to the left side of his body.

An employee was operating a rough terrain forklift to repair a metal guardrail along the highway when the forklift drove off a 6-foot ledge and rolled over. The employee suffered dislocated vertebrae, a fractured left eye socket, lacerations, and contusions.

An employee was walking next to a pipe that was being moved with a forklift when the synthetic strap slipped off the pipe and one side of the pipe fell onto the top of the employee’s foot behind the protective steel toe. The employee sustained a dislocated big toe and open and closed fractures.
