Fractures and burns

An employee was lighting a cutting torch to remove an out-of-service tank when a second tank ignited. The employee suffered a broken leg and second degree burns to the face.

An employee was standing on a pallet being lifted by a forklift to change out a light fixture. He tried to cut into a live cable with wire cutters and was shocked. He then lost his balance and fell 20 feet onto metal pipes below, suffering a head fracture, shoulder fracture, and left hand burns.

While standing on a ladder and installing light fixtures, an employee touched live wires and was thrown from the ladder. The employee suffered a fractured tailbone from the fall and electrical burns to the right thumb and left shoulder from the live wires.

An employee was clearing the hydrolyzer when backpressure caused hot feathers to discharge onto the employee. As the employee moved out of the way, he fell six feet breaking a bone over his left eye and suffering first and second degree burns to the hands, arms, face and neck.
