Report: Struck by dislodged flying object, particle at Associated Diversified Services, Inc. in Terral, Oklahoma

An employee was connecting a digger/derrick hoisting hook to a material trailer. The digger/derrick started picking up the trailer and the hook slipped off. The hook struck the employee in the face causing blunt force trauma to the face including facial fractures and coma.
