Report: Struck by dislodged flying object, particle at Schlumberger Technologies Corporation in Mentone, Texas

Employees were releasing pressure from a well, moving lines from one well to another and placing a cap on the well-pumping lines. A crown valve was open and blew up. One employee sustained a broken right forearm, a second employee sustained a head injury and a third employee sustained an injury requiring surgery.
