An employee was in the process of cleaning and setting a 20′ steel siderail for a mold while standing on top of the 2.5′ high mold. The employee stuck a piece of rebar into a hole of the mold to adjust the siderail’s position in the mold. The employee pulled on the rebar and it came out of the hole, causing the employee to fall to the ground and fracture the right femur.
