An employee was standing on top of a masonry wall, removing the wall forms, when a wall form fell. The employee’s personal protective equipment was attached to the form. As the form fell, it also brought the employee down 10 feet to the ground below, crushing the employee’s left hand, fracturing the spine, and causing lacerations.
