Report: Ignition of vapors, gases, or liquids at A & P Oil And Gas Company, Llc in Madisonville, Texas

On February 26, 2016, an employee had unloaded a load of salt water from the trailer before bringing it to the shop to help a mechanic remove bolts from the lid on the first hatch to replace the scrubber ball. They tried using vice grips and wrenches, but were unsuccessful. The employee proceeded to use a metal die grinder to attempt to remove the bolts while the mechanic walked to the tool room. Both the employee and the mechanic reported that they heard a whistling sound. At some point the vapors left behind after unloading the salt water ignited causing a flash fire. The employee’s left forearm was burned.
