Report: Fall or jump from vehicle in normal operation, nonroadway at Associated Environmental Industries in Choctaw, Oklahoma

An employee was moving soil and debris over a pile with a dozer. The equipment went nose down over an earthen embankment and slung the employee out. He struck the ground, and the equipment landed near/on top of him, lacerating his head and damaging his neck vertebrae. He was hospitalized. No seat belt was worn at the time of the incident.
