Report: Fall on same level due to tripping over an object at Caterpillar Inc. in Victoria, Texas

An employee was walking in an assembly building.
A steel cover plate covering a one-foot channel, which runs the length of the building, was raised about two millimeters above the floor surface. The employee’s foot became caught on the steel plate, causing the employee to fall on the left arm and shoulder. The employee suffered a fractured left humerus.
