Report: Fall on same level due to slipping at Cat Spec in Houston, Texas

An employee had entered a confined space in a reactor when he slipped on catalyst support media. As he fell, catalyst support beams (fire brick stacks) weighing approximately 80 to 100 pounds also fell and struck his legs, fracturing his right ankle, right fibula, and left fibula.
