Soreness, pain, hurt-nonspecified injury

After recharging the furnace, there was an explosion inside the furnace. An employee was hit by a hot piece of scrap metal that flew out of the furnace while he was using the lance. The scrap metal struck the employee in the left shoulder near the arm pit.

Two employees were fixing and troubleshooting a machine. One of them was on a ladder on top of a belt that needed to be replaced, 3.5 feet high. The machine was activated and the belt moved the ladder, causing the employee to fall to the ground and injure his lower right leg.

An employee was tasked with operating the rotary blast machine. While walking to his work station, he tripped on a piece of banding and fell to the floor, injuring his right hip. The employee was hospitalized and had surgery.

While pumping fluid out of a pit into a vacuum truck, an employee slipped from the operator’s platform and hit his knee on the ground. The employee suffered a knee injury that required surgery.

An employee was in a scissor lift using a sheetrock knife to cut sheetrock. He was putting the knife on the tray when his arm hit the cable tray causing the knife to ricocheted out of his hand and strike his left eye causing damage to the retina.

An employee was standing next to a mini excavator being used to push PVC pipe under a golf cart path when the PVC pipe shattered. He was struck in the left eye by a piece of the shattered PVC pipe.

An employee was moving a reel with a crane. The crane topped over and the employee fell out, suffering a shoulder injury. He was hospitalized.

An employee was installing wiring for new screens inside a canopy. An access door gave way and she fell through it, landing 17 feet below and suffering facial and arm trauma that required surgery.

An employee stepped up on the second step of a step ladder. Some screws/the connector had broken off the ladder, causing it to collapse. The employee fell to the floor, suffering a back injury.

An employee was being lifted in a man basket and tried to hook up a cable to a construction truck bucket. He asked the forklift operator to raise him higher and his finger was pinched between the man basket and a bolt, causing a deep tissue injury that required surgery.
